This module provides the opportunity to reflect on nursing care for individuals and families who live with chronic illness.

 

Module Objectives

  1. Address at least three factors that impact the delivery of safe and effective care in situations of chronic illness.

 

  1. Reflect on the impact person and family centered care in chronic illness situations may have on healthcare outcomes.

 

There are no new assigned readings.

 

Assignment:

 

Respond to the following questions adhering to the rubric found in Appendix C.  Please submit your responses in the assignment box.

 

  1. Explore the significance of person and family centered care in chronic illness situations, relating it specifically to a minimum of two provisions from the Code of Ethics for Nurses with Interpretive Statements.

 

  1. How can you, as an individual nurse, implement the nursing process to provide safe and quality care for those living with chronic illness? At a minimum discuss:

 

  • the significance of a thorough assessment that addresses the factors that contribute to the nursing  concern (that is, looking at the root causes of the nursing concern)

 

  • continuity of care

 

  • effective collaboration

Please adhere to the Focused Reflections grading rubric in Appendix C to earn as many points as possible.
