Week 7 Discussion: The Patient Centered Medical Home Model

Familiarize yourself with the Patient-Centered Medical Home (PCMH) model. Use these websites or research them on your own.

Respond to the following prompts using at least TWO reliable, evidence-based, and professional resources integrated with your personal experience. It is recommended that students do not read peer posts before submitting their initial discussions. Your Initial Main Discussion Thread should include the following:

  1. Describe the PCMH model also known as a health home.
  2. How is PCMH designed to improve access to care increase quality and reduce costs?
  3. Identify one of the health care delivery settings discussed in the assigned reading (e.g. home health/hospice, long term care, the VA system, retail/nursing clinics, community health centers) that you are interested in learning more about.
  4. Research & consider ways in which your selected health care delivery setting could incorporate aspects of the PCMH model to make positive change.
  5. Specifically address access, quality, and cost.
