Title: Managing Newly Diagnosed Patients with Congestive Heart Failure (CHF): Using a Patient Centered Education Program to Increase Knowledge and Prevent Exacerbation
Background: Congestive Heart Failure (CHF) is a prevalent chronic condition associated with substantial morbidity and mortality. Patients newly diagnosed with CHF often face challenges in managing their condition, leading to frequent exacerbations and hospital readmissions. Patient-centered education programs have shown promise in improving self-management and reducing exacerbations in various chronic diseases. However, the effectiveness of such programs for managing newly diagnosed CHF patients remains underexplored.
Methods: This quasi-experimental, pre-posttest study aimed to evaluate the impact of a patient-centered education program on knowledge enhancement and CHF exacerbation prevention among newly diagnosed patients. A purposive sample of adult CHF patients from [Name of Hospital/Clinic/Care Facility] was enrolled between [Start Date] and [End Date]. Participants underwent an individualized education program comprising comprehensive sessions on CHF understanding, medication management, diet and fluid restriction, physical activity, symptom recognition, and self-care strategies.
