After studying Module 6: Lecture Materials & Resources, do the following.

Post your fictional SOAP note, and For your two peer responses, reply once to each of the other SOAPs.

For this assignment, you will write a SOAP note on any of the systems we have mentioned in this course. This exercise aims to help you develop the critical thinking and documentation skills necessary for accurate and effective patient care.

Guidelines:

  1. Choose a patient: You can choose a real patient you have encountered during clinical rotations (with appropriate de-identification to protect patient privacy) or create a fictional patient scenario.
    • Develop a comprehensive patient history: Include relevant demographics, chief complaint, history of present illness, past medical history, family history, social history, and medication list.
    • Conduct a thorough physical examination: Document your findings in a clear and concise manner, using appropriate terminology and anatomical landmarks.
    • Formulate a differential diagnosis: List and briefly discuss at least three possible diagnoses for the patient’s presenting complaint.
    • Develop a comprehensive plan of care: Outline your diagnostic and therapeutic plans, including any necessary consultations, referrals, or follow-up.
  2. Document your findings in a well-organized SOAP note format:

Submission Instructions:

  • Your initial post should be formatted in order of the questions.
  • Provide support for your work from at least 2 academic sources less than 5 years old.
  • You need to respond to at least two of your peers by responding to the prompts associated with their case.
  • All replies must be constructive and use literature where possible.
  • Please post your initial response by 11:59 PM ET Thursday, and comment on the posts of two classmates by 11:59 PM ET Sunday.
  • You can expect feedback from the instructor within 48 to 72 hours from the Sunday due date
