Drug Name: Enoxaparin Classification: Anticoagulant Normal Adult Dose, Route & Schedule: The usual adult dose is based on weight and ranges from 1 mg/kg to 1.5 mg/kg, given subcutaneously once or twice daily. Indications for Use and Expected Actions: Enoxaparin is used to prevent or treat blood clots. It works by preventing the formation of clots in the blood. Side Effects/ Adverse Reactions: Common side effects include bleeding, bruising, pain or redness at the injection site, and skin rash. Serious side effects may include severe bleeding or allergic reactions. Drug and Food Interactions: Enoxaparin may interact with other blood-thinning medications, aspirin, nonsteroidal anti-inflammatory drugs (NSAIDs), and some antibiotics. Food does not affect the effectiveness of enoxaparin. Nursing Administration Considerations: Enoxaparin is given by subcutaneous injection. The injection site should be rotated to prevent irritation. The medication should be used with caution in patients with liver or kidney disease or a history of bleeding disorders. Client education & Evaluation of Medication Effectiveness: Patients should be instructed on proper injection technique and the signs of bleeding or allergic reactions. The effectiveness of the medication can be evaluated through blood tests to monitor clotting times.

Drug Name: Lorazepam Classification: Benzodiazepine Normal Adult Dose, Route & Schedule: The usual adult dose is 1 to 2 mg given orally two to three times daily or as needed. Indications for Use and Expected Actions: Lorazepam is used to treat anxiety, insomnia, and seizure disorders. It works by slowing down the activity of the brain. Side Effects/ Adverse Reactions: Common side effects include drowsiness, dizziness, and weakness. Serious side effects may include respiratory depression, confusion, and memory problems. Drug and Food Interactions: Lorazepam may interact with other medications that cause drowsiness, such as alcohol, opioids, and antihistamines. It should not be taken with grapefruit or grapefruit juice. Nursing Administration Considerations: Lorazepam is given orally or by injection. The medication should be used with caution in patients with liver or kidney disease, a history of addiction or substance abuse, or a history of depression or suicidal thoughts. Client education & Evaluation of Medication Effectiveness: Patients should be instructed on the proper use of the medication and the signs of overdose. The effectiveness of the medication can be evaluated through improvement in anxiety, insomnia, or seizure control.

Drug Name: Hydromorphone Classification: Opioid Analgesic Normal Adult Dose, Route & Schedule: The usual adult dose is 2 to 4 mg given orally every 4 to 6 hours as needed for pain relief. Indications for Use and Expected Actions: Hydromorphone is used to treat moderate to severe pain. It works by binding to opioid receptors in the brain and spinal cord. Side Effects/ Adverse Reactions: Common side effects include dizziness, nausea, and constipation. Serious side effects may include respiratory depression, hypotension, and addiction. Drug and Food Interactions: Hydromorphone may interact with other medications that cause drowsiness, such as alcohol, benzodiazepines, and antihistamines.
