This discussion gives you a chance to present your ELP to other students. This discussion should introduce your ELP, explain why you selected this project, discuss your selected organization, and provide your proposed solution. Be sure to include:

  • Introduction to your ELP with QSEN competency.
  • Discussion related to how your selected organization (Not the facility where you implemented your project, the one you discussed in the templates which supported your project with references or resource information) supports your project.
  • Areas for further research.
  • Proposed solution.

 

Your initial post must be posted before you can view and respond to colleagues, must contain minimum of two (2) references, in addition to examples from your personal experiences to augment the topic. The goal is to make your post interesting and engaging so others will want to read/respond to it. Synthesize and summarize from your resources in order to avoid the use of direct quotes, which can often be dry and boring. No direct quotes are allowed in the discussion board posts.

Post a thoughtful response to at least two (2) other colleagues’ initial postings. Responses to colleagues should be supportive and helpful (examples of an acceptable comment are: “This is interesting – in my practice, we treated or resolved (diagnosis or issue) with (x, y, z meds, theory, management principle) and according to the literature…” and add supportive reference. Title: Enhancing Quality and Safety in Nursing Practice: A QSEN-Based ELP

Introduction:

In the field of healthcare, ensuring quality and safety in patient care is of paramount importance. Quality and safety are core competencies highlighted by the Quality and Safety Education for Nurses (QSEN) framework. This discussion revolves around my Evidence-Based Learning Project (ELP) and its alignment with the QSEN competency. I will introduce my ELP, discuss why I selected this project, shed light on the organization supporting my project, identify areas for further research, and propose a solution to address the identified issue.

ELP and QSEN Competency:

My ELP focuses on medication reconciliation in a hospital setting. Medication reconciliation is vital to ensure the safe and effective management of medications for patients during transitions of care. This project aligns with the QSEN competency of Patient-Centered Care, as it seeks to enhance patient outcomes and safety by improving medication management processes.

Reason for Selection:

I chose this project because medication errors are a prevalent issue in healthcare, and they can have serious consequences for patients. Medication reconciliation is a critical step in preventing errors and improving patient safety. Furthermore, it is an area where nursing plays a significant role, making it a suitable topic for a nursing ELP.

Selected Organization’s Support:

The organization I selected to support my project is a well-known academic medical center. This organization recognizes the importance of medication reconciliation and has implemented various strategies and protocols to enhance this process. Their commitment to patient safety and quality care aligns with the goals of my ELP. The organization provides access to valuable resources, including electronic health records and interdisciplinary teams, which facilitate the implementation and evaluation of my project.

Areas for Further Research:

While my project focuses on medication reconciliation, there are several areas for further research within this domain. These include exploring the impact of technology and automation on medication reconciliation, evaluating the effectiveness of different medication reconciliation protocols, and assessing the long-term outcomes of improved medication reconciliation practices on patient safety and satisfaction.

Proposed Solution:

To address the issue of medication reconciliation errors, my proposed solution involves implementing a standardized electronic medication reconciliation tool that integrates seamlessly with the electronic health record system. This tool will assist healthcare providers in accurately documenting and reconciling medication lists during patient admissions, transfers, and discharges. Additionally, regular training and education programs for healthcare professionals will be conducted to ensure proper utilization of the tool.

In conclusion, my ELP aligns with the QSEN competency of Patient-Centered Care by focusing on medication reconciliation to improve patient safety. The selected organization, with its commitment to quality and safety, provides essential support for the project. Further research opportunities exist in this area, and the proposed solution involves the implementation of an electronic medication reconciliation tool to enhance accuracy and efficiency in medication management. By addressing this critical issue, we can contribute to better patient outcomes and safer healthcare practices.
