As the nurse caring for this client, your primary responsibilities include monitoring the client’s condition, administering medications as prescribed, assessing the effectiveness of treatment, providing comfort measures, educating the client and family members about the condition and its management, and ensuring the client’s safety.

To address the client’s complaints of lower abdominal pain, nausea, vomiting, and fever, you will need to monitor the client’s vital signs, including temperature, heart rate, respiratory rate, and blood pressure. You will also need to assess the client’s pain level using a pain scale and administer pain medication as prescribed by the healthcare provider.

To address the client’s urinary symptoms, you will need to monitor the client’s urine output, color, and odor. You will also need to ensure that the client is receiving appropriate antibiotic therapy as prescribed, monitor the client’s response to treatment, and report any adverse reactions or complications to the healthcare provider.

To address the client’s confusion and incontinence, you will need to implement measures to promote the client’s comfort and safety, including frequent toileting, incontinence care, and skin care. You will also need to assess the client’s cognitive status and report any changes to the healthcare provider.

To address the client’s nutritional needs, you will need to ensure that the client is following the prescribed clear liquid diet and monitor the client’s hydration status. You will also need to assess the client’s tolerance of the diet and report any concerns to the healthcare provider.

To ensure the client’s safety, you will need to implement appropriate fall prevention measures, including bed rails, frequent rounding, and assistive devices as needed. You will also need to assess the client’s risk for other complications, such as pressure ulcers or infections, and implement appropriate preventive measures.

Finally, you will need to provide education to the client and family members about the condition, its management, and the importance of adherence to prescribed treatments.
